Entitlement Caseworker/State Navigator
Full-Time, 80 hrs. per 2 week pay period
Days/Evenings, 12pm-8pm; including some weekends
Bachelor’s Degree in Social Work or other related field required. Prefer 1–3 years of experience in hospital/medical setting or long-term care. Knowledge of social services/health and welfare agency operations. Positive interpersonal and excellent communication skills required; basic computer skills required. Ability to be a flexible, self-motivated individual who is able to prioritize work requirements throughout the day and respond quickly to changing demands. Familiarity with Microsoft office applications. Complete required training by CMS (center for Medicare/Medicare Services), HPE (Hospital Presumptive Eligibility) and obtain Indiana State Certified Navigator status within 6 months of employment. Maintain certification yearly. Must display willingness to be a team member of the Social Work staff.
The Social Services Department offers services to inpatients, and outpatients. Population served includes infants, pediatrics, adolescents, adults, and geriatrics patients. Works closely with the licensed Social Worker staff, physicians, nursing staff, employees of other hospital departments, families, and members of outside agencies. Services to be provided by the Entitlement Caseworker include: identifying uninsured, underinsured, and low-income patients and assisting them with accessing available public health coverage, medication assistance and other entitlement programs to improve their health and financial well-being; providing information/referral to appropriate community resources for identified patients/families; acting as liaison with community agencies to facilitate obtaining services for identified patients; provide discharge planning services and maintaining monthly statistical reporting of activities and coordinating information with hospital financial department and/or physician offices. Performs activities relevant to assisting identified inpatients and outpatients with applying for health coverage, medication assistance and other entitlement programs. Develops and maintains working relationships with community health, welfare and social service agencies in order to better serve the needs of Dearborn County Hospital patients and the community.
