RN-Patient Centered Medical Home
Full-Time, 80 hrs. per 2 week pay period
Licensed R.N. in the State of Indiana, BSN required. BLS certification required. Knowledgeable in assessment, planning, implementation and evaluation of nursing care. At least 2 years experience in primary care setting. Primary Care or Internal Medicine specialized knowledge preferred. Primary care and case management experience preferred.
This position provides nursing care to patients and families along the health-illness continuum while collaborating with the healthcare team. The RN works in collaborative effort with certified physicians, providers, and medical assistants. Performs initial and ongoing assessments including biophysical, psychosocial, environmental, self-care, developmental, educational and continuity of care of the needs for neonates, children, adolescents, adults, and geriatric persons as applicable to the population and/or individual. Provides care management for specific patient populations, utilizing clinical expertise, communication and problem solving skills to achieve optimal clinical and resource outcomes. Works in concert with respective Practice Administrators/Office Leaders to assure adherence with CMS training standards, compliance, with accreditation requirements relating to ambulatory care, developing and implementing practice policies and procedures. Maintains patient satisfaction and customer service by using listening techniques and excellent verbal skills.
