Blood Pressure/Blood Sugar Clinics

Sponsored by DCH Home Health & Hospice, free blood pressure and blood sugar clinics are available to the general public at the following times and locations:  

First Tuesday of the month

Second Tuesday of the month
10:00 a.m. | Zion Lutheran Manor (Lawrenceburg)

Fourth Tuesday of the month

Participants should take their medications as scheduled.  Fasting for eight hours prior to the blood sugar screening provides more accurate results.  People with diabetes or those who are under a health professional’s care should consult their physician/nurse practitioner before fasting.  All participants are given their screening results which they are encouraged to share with their healthcare provider.

Breast-Feeding Class

A course designed to teach expectant parents the importance of breastfeeding and proper breastfeeding techniques.  The class is taught by Certified Lactation Counselors.

Cardiac Rehabilitation (Phase II) Classes

A series of classes with coordinated instruction and exercise for patients who are recovering from a heart attack, bypass surgery or other related cardiac incident or procedure. Patients with a cardiac diagnosis, such as stable angina, are also appropriate for the class.

Childbirth Education

A basic course of the physiological, physical and emotional changes in a woman's body during pregnancy.  This course will also provide information on exercise and nutrition as well as preparing for childbirth.  Pre-registration is required.

Diabetes Education Classes

For many people, living with diabetes can be overwhelming and challenging, but it doesn’t have to be.  DCH’s Certified Diabetes Educators are here to help you on your journey as you manage your diabetes.

The following classes are taught by Certified Diabetes Educators and are recognized by the American Diabetes Association as meeting the national standards for diabetes self-management education. A physician’s referral is required and there is a fee to attend.  Most insurance companies will pay for diabetes classes with a physician’s order.  Please verify coverage with your insurance company ahead of time.

We encourage you to bring an adult support person.

Living with Diabetes

Living with Diabetes is a four class series conveniently offered during the day or evening for adults with newly diagnosed type 2 diabetes who seek introductory information as well as for those who want helpful healthy living reminders. Topics include healthy meal planning, medication management, being active, monitoring and managing blood sugar, and preventing diabetes related problems.

Diabetes Management Review

A two hour group review class held on select Tuesdays and taught by Certified Diabetes Educators. This class is for adults with previously diagnosed diabetes in need of diabetes management review. Topics include ways to better manage diabetes, healthy eating, taking medications and monitoring blood sugar.

Living with Gestational Diabetes

This class is for expectant mothers with gestational diabetes. It meets on select Tuesdays and Thursdays. Topics include ways to better manage diabetes, healthy eating, taking medications and monitoring blood sugar.

Moving Towards Control

Moving Towards Control is a support group for people with diabetes or pre-diabetes. The group meets quarterly from 5:30 to 7:00 pm.

Pulmonary Rehabilitation (Phase II) Classes

A series of classes combining physical reconditioning and self-management education for patients diagnosed with chronic lung disease. Patients with a pulmonary diagnosis such as asthma, chronic bronchitis, emphysema, pulmonary fibrosis and sarcoidosis are appropriate for the class.  Participation requires a physician's order.

Smoking Cessation

The DCH Smoking Cessation program is based on the Cooper/Clayton Method and covers all aspects of smoking as well as focused education and support.

Our facilitator has been trained to assist in easing one’s journey from being a smoker to a nonsmoker through behavior modification and relapse prevention strategies.

Did you know?

  1. For every 100 smokers who try to stop smoking by going cold turkey ... 5 SUCCEED.
  2. For every 100 smokers who try to stop smoking by using nicotine replacement products ... 9 TO 15 SUCCEED.
  3. For every 100 smokers who try to stop smoking by participating in the Cooper/Clayton group method ... 40 TO 45 SUCCEED.

Weight Loss Program

The new Eat Healthy, Be Active workshop is a four class series designed for those who want to lose weight and lead a healthier lifestyle.

Classes will include nutrition tips, meal planning strategies, and recipe ideas as well as the opportunity to sample new foods at each class. The 4th class will focus on your success and will address any speed bumps you may have encountered along the way.
