BENEFITS OF HOME HEALTH CARE
• It keeps the family together by allowing the patient to receive necessary care at home.
• It provides continuity of care from the hospital or nursing facility to the patient’s home.
• It can reduce the time a patient must spend in a hospital or other health care facility.
• It promotes independence by providing the patient and their family with health related education, guidance and
support.
HOSPICE
Hospice is an approach to care which helps people with life
limiting illnesses maintain quality and dignity in their lives.
Hospice staff members and volunteers understand the great stress
placed on patients and their families and friends during this
difficult period. These individuals combine skill and compassion
to support patients and families so they may find comfort and
strength. Hospice care is directed by the patient’s needs and
wishes.
While the goal of
Hospice is to provide care for the patient at home, whether that
be a private residence or a long-term care facility, there may
be times when a brief hospitalization for symptom control may be
necessary. For these times, private rooms will be available at
Dearborn County Hospital with visiting hours tailored to meet
the patient’s medical and emotional needs.
Hospice services
are provided as a team effort on an intermittent basis by
nurses; personal caregivers; social workers; members of the
clergy; speech, physical and occupational therapists; and
specially trained community volunteers. Their responsibilities
are coordinated by the program director who receives orders from
the patient’s physician.
